Van Hohenheim is the father of Edward and Alphonse Elric. He grew up as a slave, educated in alchemy, and was used to create a Philosopher's Stone which made him immortal.

Regardless of the source material, both the anime and manga feature Hohenheim as the husband of Trisha Elric and the father of Edward and Alphonse. He is a powerful alchemist and was away from home throughout much of the time Ed and Al grew up which earned Hohenheim the hatred of Ed for not being with the family during major times, such as the death of Trisha.
 

First Anime Series

Hohenheim makes his appearance late in the series first meeting up with Winry Rockbell and confusing her for the girl's dead mother, Sara. Being several centuries old, Hohenheim was the original creator of the first Philosopher's Stone he created from the souls of those affected by plague and condemned as witches. The Stone's creation left him dying and his former lover Dante used the Stone to transfer Hohenheim's soul to another body. The two winded up transferring their souls from body to body for many years. At one point, the two had a son who died of mercury poisoning. After a failed human transmutation to resurrect the dead son which led to Envy's creation, Hohenheim regretted what he did and fled leaving Dante behind and severing any ties he had to her. Keeping himself in hiding, Hohenheim fell in love with Trisha and left his family shortly after Al's birth. Hohenheim had thought that staying in hiding would get rid of Dante's influence.
 
But upon hearing of his sons being targeted by Dante, Hohenheim came out of hiding to confront her. But upon seeing Sloth's resemblance to his dead wife, this distraction led Dante to send her former lover through the Gate of Alchemy separating his mind, body and soul. But Hohenheim managed to reunite himself and traveled to the other side of the Gate to the later years of World War I era London. He became an adviser to prime minister Winston Churchill and later traveled to Germany siding with the Thule Society in hopes of finding a way to get Ed back to his world when his son made himself a sacrifice to the Gate to reunite Al's being.

Hohenheim makes a couple appearances in the movie's major scenes where he becomes a sacrifice with Envy's dragon form to open the Gate of Alchemy from Germany's side of the Gate.
 

The Manga

While still several centuries old and having a connection to the main antagonist, Hohenheim's origins are different in the manga. Being a slave known as "Number 23" for a Cselkcess scientist, the man who would later become Hohenheim came across a flask while mopping his master's laboratory one day. A creature in the flask, being the first homunculus and created with Hohenheim's blood, gave Hohenheim his present name and knowledge as a sign of gratitude for his creation. Thanks to the homunculus's teachings, Hohenheim rose to be his master's assistant greatly improving his life.
 
Later on, the homunculus tricked the king of Cselkcess into creating a large transmutation circle surrounding the entire country promising him it would lead to the secret of immortality. With Hohenheim and the homunculus in the center of the completed circle, the entire country's population was wiped out and Hohenheim is brought before the Gates of Truth. Upon being brought back to the now lifeless country, Hohenheim discovers the homunculus had took on a copy of his body. As a "gift" for helping him, the homunculus grants Hohenheim immortality by fusing his body with the many souls that were sacrificed in the transmutation circle turning him into a living Philosopher's Stone.
 
Despite his status as a "monster," Trisha accepted Hohenheim as the two fell in love and brought up Ed and Al. He left his family to search for a means to become mortal so he could live and die with his new family as well as confront the homunculus who granted him his talents now known as Father. 
 
Returning years later to Resembol year later with little accomplished, he discovers his house burnt down, an angered Ed at Trisha's grave and the news of Ed and Al's failed attempt to resurrect her. Afterward, he continues journeying where he encounters Rose, the homunculus Sloth and Izumi Curtis. He heals up Izumi's ravaged internal organs by reordering them in a proper arrangement. He later meets up with Al in Lior where the two have a heart-to-heart talk. He also meets up with Ed again where after getting punched by his son, Hohenheim explains his past. 
 
During a later battle with Pride, Hohenheim creates an earthly prison that seals up Al and the shadow-manipulating homunculus to prevent Pride from inflicting any further harm. At this point, he becomes the leader of the expedition to take down Father and mentions he had a plan to counteract Father's transmutation circle as a last resort. Reaching Father's lair, he has two teams set up where he teams up with Ran Fan, whom he later allows to go when seeking out Ling Yao/ Greed.  
  
Hohenheim confronts Father one-on-one where their battle eventually leads to Hohenheim being stabbed. As Father attempts to extract the Philosopher's Stone from Hohenheim's body, the many souls that make the Stone attack Father wiping out his human body. However, Father survives the attack and emerges in his homunculus form. Father proceeds to absorb Hohenheim into his body and makes use of him and the other sacrifices (those who have seen the Gate of Alchemy which also include Ed, Al, Roy and Izumi) in his grand plan to use the people of Amestris surrounded in the Nationwide Transmutation Circle as sacrifices to absorb a being known as "God" and acquire ultimate knowledge and power.
 
Gaining what he wanted, Father attempts to kill off the remaining human survivors who have met his use as sacrifices by shutting down their ability to use alchemy and attacking them with a blast of pure energy from above. Hohenheim blocks the blast from above and Mei Chang blocks Father's attack from below them. Hohenheim then activates his trump card: using the souls of the Xerxes people he stored up from his body which were hidden in the Earth, the souls lead the souls of the Amestris people back to their bodies. No longer having the necessary energy to keep "God" in his body, Father's body began to deteriorate. To add further trouble for him, Scar had succeeded in activating the Alkahestry National Circle which restored alchemy to Amestris giving the Elric brothers and Izumi the power they needed to fight against Father.
 
Having exhausted much of his Philosopher's Stone in defending his sons, Hohenheim was left to witness Al's sacrifice to restore Ed's arm and defeat Father. After Father's demise, Hohenheim offers to use up the rest of his life to bring Al back to life because he is their father. Ed scolds Hohenheim for considering the idea and finally acknowledges the man he once hated as his father while weeping. Hohenheim would later return to Resembool dying at Trisha's grave while cherishing the fact his wife and sons gave him something to appreciate in life and would be laid to rest with a grave alongside his wife.
