Rena pays Keiichi a visit and begins acting strangely, prompting Keiichi to succumb to his paranoia and hurt her. Later, Keiichi is being followed by Rena. Keiichi escapes from Rena and finds himself being chased by a mysterious group of men, who knock him out. When he awakens, he is subdued by Rena and Mion, who attempt to inject him with a syringe.

"I THOUGHT I TOLD YOU TO USE SODA?!"
Rena tries to bring Keiichi dinner, but he remembers the deadly ohagi she brought on the previous night, and lies that he has leftovers. Rena detects the lie and tries to barge her way in, but Keiichi shoves the door close and clamps her fingers in between the gap. After apologizing repetitively, Rena manages to slip free and escapes.
As Ooishi continues his conversation with Keiichi over the phone, Keiichi notices Rena standing outside his apartment, incessantly mumbling the same words repetitively, "I'm sorry, I'm sorry..."
"I should've killed that old man..."
The next day, when Mion tries to get Keiichi to stop his practice swings, he snaps back at her, claiming that friends don't hide secrets from each other, and that he knows about her fight with the dam construction workers. Mion bursts into tears, but curses Ooishi for leaking the information, that she will kill him for doing so.
After school, Rena stalks Keiichi with her giant cleaver. When he confronts Rena, she tries to convince him of Oyashiro-sama's existence, before laughing maniacally. Keiichi shoves her away and runs from her, but is instead confronted with two uniformed men. He tries to run, but the two give chase. He wards them away with his bat, but is knocked out from behind by a third uniformed man...
"Let's play Doctor..."
When he awakes, he finds Rena and Mion in his house, supposedly waiting for 'the director' to arrive. When asked about the director's identity, the two girls instead laugh maniacally. They decide to play a punishment game. Rena then holds down Keiichi as Mion pulls out a syringe and approaches him. A flashback occurs as Keiichi remembers all the good times he had with his friends, the episode then cuts to the scene that began the series - Keiichi whacking Rena and Mion's lifeless bodies with his bat.
Before Keiichi could contemplate further on the situation, Kyosuke Irie arrives in a white van with the same group of uniformed men from earlier. Keiichi removes the note he hid behind his clock and jots down further details. He also tapes the syringe behind the clock as well. Tears flow as he gives one last gaze on his beloved friends before he escapes from the backdoor. One of the uniformed men guarding the back gives chase, but Keiichi manages to slip free and gets to a phone booth.
"Draw me like one of your Japanese girls..."
At the police station, Ooishi receives a call from an incoherent Keiichi who claims that Oyashiro-sama does exist, and that it is chasing him. When Ooishi tries to get Keiichi to turn around to gain a look at his pursuer, the sound of flesh being torn open is transmitted over the receiver. Keiichi claws his throat apart and becomes another victim in the string of village curses. As Ooishi tries to get Keiichi to stop, the words, "I'm sorry, I'm sorry," become the last words he overhears...
In Ooishi's final report, he dictates that Keiichi invited Rena and Mion over to have them murdered. The clean-up team that arrives at Keiichi's house finds the memo hidden behind the clock to be torn in three parts - with the middle portion removed. Moreover, the syringe is missing. As Ooishi narrates on what Keiichi wrote, Keiichi's voice takes over, stating that his only wish is for the reader to uncover the truth.

Points of Interest
- Keiichi becomes really harsh towards his friends in this episode. Understandable when you think about the ohagi needle, but you'd feel sympathetic towards Mion when you realize the truth later and rewatch this episode...
- The same applies for Rena, who is actually quite sane throughout the whole show compared to the Big Bad of the series.
- Despite that neither party had actually hung up the phone, the dead tone sounds up during the conversation.
- In Ooishi's report, we learn that the actual name for Keiichi's residential area is called Shishibone City.
