Portgas D. Rouge was the mother of Protgas D. Ace. She risked her life to save her unborn son, but died shortly after giving birth. She is another D who died smiling.
After the death of Gold D. Roger. Rouge knew she would now have to raise their child by herself. It was on the island of Baterilla in the South Blue where Rouge was living at the time, and the Marines and World Government followed reports of Roger's presence. World had reached that them the Pirate King had spent his last year of freedom on this land and behaving as if he was a father. All women who were pregnant came under suspicion.
Rouge saw as women were being hunted all over and taken away. Looking for any sign that Roger had sired a child before his capture. Women were taken away and questioned. Orders were to kill any mother and child that seemed suspicious. In order to keep her child safe. Rouge suffered great pain and fatigue has she held her child within her body for twenty months. It was an act of extreme will and love for her unborn child. She held the pregnancy and kept it secret just long enough for the World Government to leave.
By the time her pregnancy had come to full term. She was too weakened to survive. Garp, the Marine hero, was present that night. He had made a deal to protect Roger's son. By dawn, her child was born. She held the newborn in her arms for the first and last time. She gave him the name that Roger had selected if it was a boy. Roger D. Ace. With a smile on her face, Rouge passed away. She left her son in the hands of Garp to hide and raise.
Ace would not keep the full name his mother had given him. Out of respect for her sacrifice. Ace took on her family name to become Portgas D. Ace.
