Following the true revival of the Kamen Rider Franchise in 2000 Kuuga is the first rider to have multiple forms in fact he has more then 13 forms that`s the highest count to date.
Kamen Rider Kuuga
In the original series he is Yuusuke Godai who put the Arcle belt on and transformed into Kuuga`s white growing form to fight a Grongi. Over the time he grew stronger by training he slowly but surely gained access to all of Kuuga`s forms. He is rumored to be the true Kuuga who fought Decade in the Rider wars.

Decade`s Kuuga is Yuusuke Onodera was given the belt by the mysterious Narutaki who warned him that he must fight Decade in the future, his Kuuga form is kinda powered down this might show that he only recently begun to use it. He was in love with a cop named Ai before the Grongi gas killed her now he travels the 9 worlds alongside Tsukasa, Nastumi and Kivahala. In Agito`s world (Decade) Yuusuke took the role of Kamen Rider G3-X to be with that worlds Yashiro but ultimately decided to continue to travel with Decade and co after realizing that Yashiro loved that world`s Shioichi (Agito). His surname is literally taken from Shatoro Ishinimori`s birth name Shotaro Onodora.
Ancient Kuuga
The original Kuuga who fought the Grongi tribe on the side of the Linto(Lords) in ancient times he has been shown in Kuuga a few times mainly in episode 1 as both a mummy who holds the belt and as the Kuuga fighting the Grongi in Yuusuke`s visions his true identity remains unknown but it has shown that he had full control of the belt`s main 4 forms.
The Arcle and Kuuga`s Forms
The Arcle is a ancient belt that a civilization called the Linto used to fight the Grongi in the times of old this is a powerful device allowing the use to gain 11 different types of armor that always suited the time and place of battle and the power to change harmless objects in to weapons. In Kamen Rider Decade Kuuga gains access to a new 12th form that of Kuuga Gouram he just needed Decade to activate the power but after that he could change in to that form himself

Rising forms:
The Rising forms are upgrades of Kuuga`s basic forms who all gain several new golden accents. the belt gets a new golden add-on placed on the buckle and the Mighty form gets a new leg ring. He has the Rising Mighty Form, the Rising Dragon Form, the Rising Pegasus Form and the Rising Titan form. These forms are not only more powerful but the weapons are also upgraded aside from those upgrades Kuuga also has the Amazing Mighty Form which is a black and gold version of his mighty form this is his second most powerful form.

In the 2009 series Kamen Rider Decade Yuusuke Onodera will eventually receive a even more powerful form beyond his Ultimate Form which is now covered with a new more powerful golden armor. This form also come with the the evil black-eyed variant which might indicate that Kuuga will go berserk later on.
