Question about Madoka (definite spoilers)

While trying to find screen caps to use for the Top 10 article, I came across this one from episode 10:

My memory might be wrong, but didn't the blond get her head ripped off and die early in the series? Is this screen cap from a flashback or did she get brought back to life? I've always heard that this show is dark but it would be a major turn off to me if they pulled a Mai-Hime where all of this dark stuff and death happens but near the end everything bad gets completely undone and all of the meaningful sacrifices and sad deaths are erased or lessened by none of it being permanent.

Is this the case?

@sickVisionz: The picture above likely comes from a flashback of Homura (the cold, seemingly emotionless girl) recalling her time with Madoka and the other mahou shoujo from alternate timelines as she has the power to go back through and freeze time. And yes, Mami (the mentioned blonde) did indeed become witch chow earlier in the series within the title's current timeline.

@sickVisionz: I don't remember that scene in particular but time travel is introduced as a plot mechanic and at the end of the first season Madoka becomes essentially omnipotent.

@sickVisionz: Yes, it was confirmed as a flashback from one of the many other time frames Homura had to endure in order to save Madoka from her death, which was why Homura wished for time-traveling powers.

@sickVisionz: Everyone else answered the part about what happened with the blonde, Mami, so yeah.

Also, let me reassure you that they definitely don't pull a Mai-HiME. There is an exactitude with which the writer (Gen Urobuchi) approached the progression of the plot in Madoka, so everything is resolved much more competently, which is partly why Madoka ended up in a lot of top lists.
