There's only one manga I've really read that has had a death in it that surprised me (just like in the anime) and that was Teresa from Claymore. That surprised me and left me with my jaw dropped.

Itachi Uchiha's death was really heart breaking :'(

L's death...while shocking, I didnt really think was that good of a death. Surprising for the character, yeah, but he deserved something pwn filled :(

Let's see....my real favorite death though, Ive got to say....hmmm

Probably would be ruro ken side character number 96, the one that Saitou stabbed through the back of the mouth and into his head :)
