Set two years after the events of the game, Cloud Strife and his friends must discover the mystery of the plague called "Geostigma".

Two years after the events of Final Fantasy VII, a new city called Edge has been created. Cloud has been living there with Tifa, Marlene and an orphan boy called Denzel, who suffers from a strange disease called "Geostigma". When Cloud is returning home he is attacked by three men, Kadaj, Loz, and Yazoo, who are in search of their "mother".

Cloud survives the battle and meets with the Turks, only to find out that Rufus Shinra is still alive. Rufus tries to convince Cloud to work with him and stop the trio, an offer that Cloud refuses. After this, he is confronted by Kadaj, the leader of the group, who demands Rufus to give him his "mother". During their conversation it is revealed that the "mother" is Jenova's remains.

Meanwhile, Loz goes to Aerith's church to try to find the remains and is confronted by Tifa. During the battle, he receives a message from Kadaj, who tells him he must kidnap Marlene, which he does.

The trio kidnaps a group of kids infected with the geostigma and take them to the Forgotten City, where they brainwash the kids. Cloud attempts to save them but fails and is rescued by Vincent, who tells him that the trio's plan is to revive Sephiroth.

Cloud goes back to Edge to face Kadaj, while his friends take care of the monsters attacking the city and Reno and Rude fight Loz and Yazoo. Kadaj retrieves Jenova's remains from Rufus and escapes, but Cloud pursuits him. They start fighting across different places until finally arriving to the Shinra Headquarters.

They continue fighting until Kadaj absorbs Jenova's remains, transforming into Sephiroth. He reveals his plan of controlling Geostigma and then use the planet as vessel to travel is space and find new planets for him to rule. Cloud fights Sephiroth and defeats him, leaving a weakened Kadaj. Aerith begins to pour healing rain across Edge, curing the people who suffer from the Geostigma and then takes Kadaj to the life stream. Then Cloud is attacked by Loz and Yazoo who self-destruct themselves in order to kill him.

Afterward Cloud appears surrounded by white light, Zack and Aerith tell him that he is not meant to be there yet and take him back to Aerith's church, where he reunites with his friends.
