Charlotte

Origin

Charlotte became one of the witches that drove people to commit suicide in the series Puella Magi Madoka Magica. Like the other witches, she doesn't have a clear history about her past. In the series, she was mainly hunted by Mami Tomoe in battle, but somehow survived her "Trio Finale" attack and counterattacked with her second form successively. Charlotte was later killed by Homura Akemi after beheading Mami.

Charlotte's Second Form

The first form of Charlotte is presumed to be her familiar apperance when it was really the first part of her witch form. The second form is much bigger and appears like a colorful worm with small wings.
