Arashi Mikami is a member of Team Ampoule One in Black Label. He barely survived a terrorist attack as a young boy, and his life was saved by surgically implanting the body parts of his childhood friend.
As a child, Arashi Mikami was the best friend with Ryu Mochizuki, the son of Dr. Mochizuki. On the say of the terrorist bombing at the International Medical Confrence in Bioka City. Only Arashi and Ryu were the only survivors. Much of Arashi's body was destroyed when he sacrificed himself to save his friend Ryu by acting as a human shield. Though he had saved his friend body, a head injury resulted in brain death. Dr. Mochizuki was forced to place the black label on his own son, but used his body to restore Arashi's.
His first appearance was in Triage X Volume 1 CH. 1 "CASE: 1 Prescription from Hell I".
Arashi Mikami is a handsome but socially awkward young man and is often introspective as his mind lingers on this past. Even as a young boy he showed a courageous side by trying to save his childhood friend at risk of his own life. Much of his body is covered in massive scars due to the surgeries he underwent to save his life. He was Ryu's limbs and heart. Because his life was extended at the cost of his friend's body. He is filled with a sense of both guilt for the past but also intense loyalty for Masamune Mochizuki.

On his first mission for the Black Label, Arashi was given the kill order for Hideo Aranami, a corrupt business owner of Aranami Construction Industries. He was teamed with Mikoto Kiba, his school upperclassman. They were in the back of an ambulance on motorcycle. Once he got the order from Yuko Sagiri, they shot out of the back of the ambulance and head toward the guarded iron gate. Yuko's sniper partner, Miki Tsurugi, got the gate to fall by blasting the concrete supports. As he drove through the compound and into the house, Mikoto covered him by shooting anyone in their path. She drove down the hallway she called out and turned sharply into Hideo's room. Though, Mokoto fell off from the bike and into the hall outside. He told Hideo that he was a malignant tumor tot he world, but tossed him a pistol with one round in to gun. Laughing, Hideo aimed at Arashi and fired, but the bullet ricocheted off his helmet. Arashi then shot Hideo in the head. Mikoto burst into the room yelling at him and demanding to know what he was just trying to do. He refused to answer. She drove the bike out of the house and they were nearly overcome by an explosion that destroyed Hideo's home. Pulling up to a truck, Mikoto yelled at Oriha for using too much firepower.
Back at Mochizuki General Hospital, Mikoto continued asking Arashi about his earlier actions but he was too lost in thought to notice. He only reacted after she jumped on top of the lockers, yelling. Afterward, he and the rest of the Black label met in the room of Masamune Mochizuki. Arashi left the hospital once the meeting was done and sat at a bench as he was hit with pain. He took a few pills from his coat and looked on with an angry expression.
